This article presents an action-research study on the contribution of a boundary object to communication between professionals and caregivers in the home care of frail elderly people. Conducted in four Multi-Purpose Home Care and Support Services (SPASADs), the study is based on the analysis of 53 folders and 27 interviews. The results show that while the tool facilitates communication, it largely remains confined within professional silos. However, the presence of a boundary actor strengthens its role as a mediator between the professional and lay worlds.
